Getting online


You can order repeat prescriptions, view your medical record (including test results), and book nurse and certain GP appointments, via our online service.

To register for online prescriptions & appointments service please call the practice on 0113 22 11 888 to register (we will give you a user name and password).

To register for online medical records access (including test results) then please see the below.

Once you are registered, then use the online link from the front page of this website to access all our online services.

Online Medical  Records Access

All patients who are registered with our Online Service are able to view their medical records online.

We would ask firstly that you read the information in the two leaflets below. Then to register please print and complete the request form below and bring it in to the surgery. Alternativey, if you come in to the front desk we can print you a form which is largely pre-filled. We will ask you a few questions to confirm your identity.

If you wish to apply for proxy access, for example on behalf of someone you care for, then there is a different form to complete (below). Please be aware that for children, proxy access is only allowable up until their 11th birthday, after which they have to have their own login details.. This is in line with national guidance.
