GP appointments at St Martins Practice

If you need a medical advice regarding yourself or your baby please book an appointment.

We understand parents / carers need quick medical advice for their babies & children. Appointment Request Forms for children age 0-5 will get a same day response, provided the form is received by 11am.

0 – 19 Team Public Health Nurse (formerly Health Visitors)

The 0 – 19 team is there to support you in parenting and to give advice on child development until your child goes to school.  

It offers developmental reviews for your baby/child at aged 8-12 months and again at 2-2½ years. These will usually take place either in a community setting such as a health centre or in your home by a 0-19 Family Health Worker.

the service will take your child’s height and weight an undertake vision screening. Usually this will take place in your child’s school when they are in reception and you will receive a letter about it. Prior to your child starting primary school we will send you a questionnaire. It will ask questions about your child’s health to see if there is any support you or your child may need.

Breastfeeding at the practice

Breastfeeding is welcome at the practice, while you are waiting for appointments / if you are passing & need a friendly place to breastfeed. Either in the waiting room, or if you prefer somewhere else than please ask the front desk who may be able to find you an empty consulting room or a more private waiting area away from the main waiting room. 

Immunisations

We offer a full range of childhood vaccinations. Please feel free to make an appointment with the Practice Nurse if you would like your child to have their immunisations.

Or if you would simply like to know more about the (UK) vaccines programme then do feel free to make an appointment with the Nurse or GP.

Depression and Baby Blues

The Majority of women experience mood swings and are often tearful during the first few days and weeks after having their baby. However in some women these symptoms do not disappear, or become more severe. 

Support and understanding from family, friends, and sometimes from a professional can help you to recover. Other treatment options are also available. Your health visitor and GP will discuss how you are feeling when they see you. Sometimes a 10 question screening questionnaire is used to help assess how you are feeling.

Advice for parents about common childhood conditions:

The Healthier Together website has been written by the NHS and the Royal College of Paediatrics & Child Health. It has clear, easy to use information about common illnesses & problems experienced by babies, children & young people. There is a widget you can load onto your homescreen.

The Little Orange Book contains advice & tips on how to manage common illnesses & problems that babies & young children often experience in the first 5 years of their lives. It also has information on more serious conditions, what to look out for & how to get help. 

When Should I Worry? is a booklet which provides information for parents about the management of respiratory tract infections (coughs, colds, sore throats, and ear aches) in children.

Cancer Screening

We provide various forms of cancer screening:

Cervical screening (a smear test) checks the health of your cervix, which is the opening to your womb from your vagina. It’s not a test for cancer, it’s a test to help prevent cancer. It’s one of the best ways to protect yourself from cervical cancer.

At St Martin’s we want to encourage women to have their screening test regularly. It’s your choice if you want to come for cervical screening but it is one of the best ways to protect you from cervical cancer. Women age 25 – 49 are invited to attend every 3 years. Women age 50 – 64 are invited to attend every 5 years. 

We have cervical screening appointments in the daytime and evening at St Martins with one of our nursing team, and also at weekends at our local hub at Rutland Lodge. Bowel cancer screening checks if you could have bowel cancer, which is a common type of cancer. Screening can help find it at an early stage, when it’s easier to treat.
Screening is available to everyone aged 60 or over. Everyone aged 60 to 74 who is registered with a GP and lives in England is automatically sent a bowel cancer screening kit every 2 years.

About 1 in 8 women in the UK are diagnosed with breast cancer during their lifetime. If it’s detected early, treatment is more successful and there’s a good chance of recovery.

Breast screening (mammogram) – NHS aims to find breast cancers early. It uses an X-ray test called a mammogram that can spot cancers when they’re too small to see or feel.

In the meantime, if you’re worried about breast cancer symptoms, such as a lump or an area of thickened tissue in a breast, or you notice that your breasts look or feel different from what’s normal for you, do not wait to be offered screening, call us.

Services from other healthcare professionals working with us

A range of services provided by additional healthcare staff are available:

The midwife is with us for 2 days a week offering antenatal care. If you are pregnant then your first appointment is for 90 minutes and our Patient Support Team can book you an appointment. (There is no need to see a GP first, in order to access routine antenatal care.)
This is a telephone appointment service for when you have medication queries and issues and need to speak to a pharmacist. For example, you are having side-effects, there are supply issues and you need an alternative. The pharmacists are able to prescribe and organise prescriptions electronically. 
We have a full time Recovery Co-ordinator on site, who will work with you if you have an addiction to alcohol or drugs. Our Patient Support Team can advise you about how to make an appointment to see Kathryn.
This is a telephone appointment service for patients who need to speak to an advanced physiotherapist regarding muscle and joint problems. They will issue advice and where appropriate arrange a follow up. Our Patient Support Team can book you directly into these telephone appointments, which are all on the weekends and evenings.

Wellbeing Services

We have a range of wellbeing services available at the practice and locally that we can introduce you to:

‘More Than Just a Coffee Morning’

Come and join us for fun and games at “More Than Just a Coffee Morning.”

Thursdays 10am – 12pm at the Polish Centre on Chapeltown Road.

Dominoes, knitting, gentle zumba and much more! Just turn up…

Carers Clinic

Carers Leeds provides one to one support to carers by offering emotional as well as practical support. They help carers navigate through the system, in order to access appropriate services and provision, including welfare benefits. The carer’s clinic is with Carrie Grey from Carers Leeds and is offered within St Martins Practice once a month on a Thursday.

Our patient support team can book you an appointment with Carrie.

Citizens Advice Leeds

Citizens Advice Leeds provides free confidential and impartial advice about money, benefits, housing or employment problems.

There is a weekly session based at our practice. To book into it please contact Citizens Advice Leeds on 0113 223 4400, or by email or webchat through their website.

Connect Well Wellbeing Service

We believe that everyone can live a fulfilled life however, sometimes people need help to achieve this. Feeling connected to your community has lots of health and wellbeing benefits that support resilience, independence and good physical and mental health.

Aparjita is our Wellbeing Co-ordinator from Connect Well who works with you to connect you to services and activities in your community to benefit your health and wellbeing.

Our patient support team can give you information about seeing Aparjita our Wellbeing Co-ordinator.

DWP Patient Coach

Kathryn is the DWP patient coach at St Martin’s, on Monday afternoons. Her role is to support you in thinking about work / volunteering and / or training. 

She can provide a personalised and supportive service to help you plan your next moves. She can also talk to you about how Universal Credit works and the support you can get from your job centre patient coach, together identifying the barriers that may be stopping you from working and plan ways of overcoming these.

Healthy Minds

If you are feeling low, irritable, anxious or stressed, are experiencing poor sleep and/or concentration or have had a recent bereavement then Jenny, the Healthy Minds practitioner, may be able to help. Jenny can also help support you if you are waiting to be seen by IaPT or have been seen previously and feel that things are getting worse again.

The Healthy Minds practitioner will call you on the phone (an appointment lasts 20 minutes) and can talk through what’s troubling you and possible solutions to the problem, directing you to other support or self-help intervention if needed.

St Martins Men’s Group

Having fun with games and conversation, plus peer to peer support, are the aims of these sessions.

There are free refreshments, including healthy snacks, and the group meets in the relaxed setting of our big room on the 3rd floor.

Some of our work is outside that agreed by the NHS so we charge for it.

The attached list is the most common items for which there will be a charge. If you are not clear about whether or not there is a charge then please ask our advice beforehand.

Travel Vaccinations

If you feel you need travel vacinations then please be aware that we do not offer a comprehensive travel health service.

However, we can advise you about which vaccinations you may need & we can give you those which are available under the NHS (no charge). In this case, please contact us online, stating that you would like travel vaccines, and an online questionnaire will be sent you to complete.

Please contact us minimum 8 weeks before travel to allow time for vaccines to become effective before departure.

The closest comprehensive travel health clinic is SF Screening which is based at Woodhouse Medical Practice.

NHS Health Checks

Who is invited for a free NHS health-check?

If you are age 40-74, and do not have a diagnosed long-term condition such as high blood pressure, diabetes or heart disease, then you will receive an invitation from us for your health-check.

I feel fine! Why do I need a health-check?

The NHS Health Check helps to identify potential risks early. By having this check and following the advice of your health professional, you improve your chances of living a healthier life.

At Risk of Diabetes and Cardiovascular Disease – Annual Reviews

You are invited for your At Risk of Diabetes annual review, because in the past you’ve been found to be at increased risk of getting diabetes, a condition where your body does not process sugar properly.

Being At Risk of developing diabetes is an early warning that you could develop diabetes in the future. Uncontrolled diabetes is a serious and life-limiting illness. Lifestyle changes such as increasing your exercise and making changes to your diet can postpone, or entirely prevent, developing diabetes.

The review is a 20 minute appointment with one of our Clinical Support Workers. They will take a blood sample to monitor your blood sugar so we can help you stay well and active.  

It is an opportunity to discuss any changes you have been able to make, and to discuss any barriers you have found and ways to overcome them.

You will be advised about other sources of support and advice to move more, food & cooking. Small changes can significantly reduce your risk of developing diabetes.

It is aimed to give you real peace of mind. It’s important to come along even if you feel well. The At Risk Review can help you to stay well. Simple changes can reduce your risk of developing Cardiovascular Disease.
