Nursing Team

Practice Nurses

Practice nurses are qualified and registered nurses. They can help with health issues such as family planning, healthy living advice, dressings, cervical cancer screening.

The practice nurses run clinics for long-term health conditions: asthma, lung disease, diabetes, and heart disease. They follow up with patients after certain routine blood tests including raised HBA1C (sugar levels), and if tests show that you have a raised risk of cardio-vascular disease.

Clinical Support Workers

Clinical Support Workers carry out tasks such as phlebotomy (drawing blood), ECG tests, hypertension annual reviews (annual review of high bloods pressure), NHS health checks & certain dressings. They also carry out flu vaccinations and routine B12 vaccinations.

Non-urgent advice:  Community / district nursing team

Based in Chapeltown at Tribecca House, the community nursing team provide nursing care for elderly and vulnerable people, and people with disabilities, in their own homes

Non-urgent advice: Pharmacists

Our team of practice based pharmacists are medicines experts. They work within the clinical team to resolve day-to-day medicine issues and also consult with patients directly for medication reviews. This includes providing help to manage long-term conditions, advising those taking multiple medicines (polypharmacy) and delivering clinical advice about treatments.

Non-urgent advice: Social Prescribing Team

Our social prescribing team supports people with a wide range of social, emotional or practical needs including housing, food, employment & skills, friendships & relationships.

The practice manager is involved in managing all of the business aspects of the practice such as making sure that the right systems are in place to provide a high quality of patient care, human resources, finance, patient safety, premises and equipment and information technology. The practice manager supports the GPs and other medical professionals with delivering patient services and also helps to develop extended services to enhance patient care.

Our valuable Patient Support team is: Barbara, Bradley, Christina, Clare, Jacob, Jahnvi, Sara and Pamela. The team is your first contact for all enquiries and appointment booking.

They go through a comprehensive training programme to ensure that they are knowledgeable and skilled to give you the best possible service. When you ring to book an appointment, our team is trained to ask for a brief reason for the appointment so they can sign-post you to the person or service best able to help you.

Patient Support can also provide you with information on our services and your test results. They also perform other important tasks such as issuing repeat prescriptions and dealing with prescription enquiries, dealing with financial claims, dealing with patient records and carrying out searches and practice audits.
