You are invited for your At Risk of Diabetes annual review, because in the past you’ve been found to be at increased risk of getting diabetes, a condition where your body does not process sugar properly.
Being At Risk of developing diabetes is an early warning that you could develop diabetes in the future. Uncontrolled diabetes is a serious and life-limiting illness. Lifestyle changes such as increasing your exercise and making changes to your diet can postpone, or entirely prevent, developing diabetes.
The review is a 20 minute appointment with one of Health Care Assistants Ellie or Jo. They will take a blood sample to monitor your blood sugar so we can help you stay well and active.
It is an opportunity to discuss any changes you have been able to make, and to discuss any barriers you have found and ways to overcome them.
Ellie and Jo can advise you about other sources of support and advice to move more, food & cooking. Small changes can significantly reduce your risk of developing diabetes.
It is aimed to give you real peace of mind. It’s important to come along even if you feel well. The At Risk Review can help you to stay well. Simple changes can reduce your risk of developing Cardiovascular Disease.
